Birth Announcement Form

• Must be no more than one month from the time of birth.

• Deadline is noon on Monday.

• If you would like additional information included, a paid announcement may be placed and would appear just as you wish, including a photograph.

• Must include a daytime phone number and name of person submitting information.

• If you would like other information printed or a picture, this can be arranged for as a paid announcement.

• The notice will run free of charge, if the information is submitted within 30 days of the child's birth.
